What is the difference between CrCl and GFR?

Creatinine clearance (CrCl) measures how quickly the kidneys remove creatinine from the blood and is calculated from serum creatinine, age, weight, and sex. Glomerular filtration rate (GFR) is a direct measure of kidney filtration capacity, usually reported as eGFR (estimated GFR) using the CKD-EPI or MDRD equations. Both are proxies for kidney function. Cockcroft-Gault CrCl is preferred for drug dosing because most drug package inserts were developed using that formula. CKD-EPI eGFR is preferred for CKD staging and epidemiology.

Why does the equation correct for females by multiplying by 0.85?

Creatinine is produced by muscle metabolism. On average, women have less skeletal muscle mass than men of the same height and weight, so they produce less creatinine per day. The 0.85 correction factor (a 15% reduction) approximates this average difference. Note that this is a group average - highly muscular women or women with very low muscle mass (e.g., sarcopenia) will have results that diverge from the population norm.
