Frequently Asked Questions

What happens if I skip a dose?

If you miss a weekly injection and remember within 5 days, take it as soon as possible and resume the regular schedule. If more than 5 days have passed, skip the missed dose and continue with the next scheduled dose. Do not double-dose. If you miss several doses, consult your prescriber before resuming, as you may need to restart at a lower dose.

Is the weight loss permanent after stopping?

Large regain after stopping is common. The STEP-4 trial found that approximately two-thirds of weight lost during semaglutide treatment was regained within 1 year of stopping. This reflects the medication's ongoing biological action on appetite and energy regulation, not a dependency in the pharmacological sense. Most current guidelines consider these medications as long-term treatments, similar to antihypertensives.

Can I take these medications if I have a personal or family history of thyroid cancer?

GLP-1 receptor agonists carry a boxed warning for a risk of thyroid C-cell tumors based on animal studies. They are contraindicated in patients with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Discuss your personal and family medical history fully with your prescriber before starting.

Why do I need to titrate slowly? Can I go faster?

Standard 4-week titration is designed to minimize nausea and vomiting, which are the most common reasons for early discontinuation. Faster titration significantly increases the risk of intolerable side effects. Some prescribers allow longer time at each step if side effects are present, or in the opposite direction for patients who are tolerating each step well, but any modification to the schedule should be directed by your prescriber.
